Model patient requirements:
PLEASE NOTE: If you are accepted as a model patient, there will be a $25 deposit made at the time of appointment which will be applied to any heavily discounted treatment you may receive. This $25 fee will be forfeited if you do not give us 24 hour notice of cancellation by email or you do not show up for your appointment. We look forward to welcoming you to our training center!
