Who Can Inject in Massachusetts?

In Massachusetts, Botox and dermal filler treatments are regulated injectable procedures. Eligible providers generally need the correct healthcare license, proper training, and authority to perform the treatment within their scope of practice. Massachusetts also places limits on where these injections may be performed and prohibits authorized injectors from delegating Botox or filler injections to people who are not allowed to perform them.

Here is a breakdown of the eligible medical professionals and their Massachusetts state requirements:
Profession
Eligibility
Massachusetts State Requirements
Dentists (DDS / DMD)
Eligible
Massachusetts dentists may administer Botox and dermal fillers when treatment is part of the patient's comprehensive dental treatment plan and remains within the practice of dentistry. General dentists must successfully complete at least 8 hours of training in botulinum toxins and/or 8 hours in dermal fillers, including hands-on instruction, through ADA CERP, AGD PACE, or another nationally recognized accredited entity approved by the Board.
Physicians (MD / DO)
Eligible
Licensed physicians may evaluate, prescribe, and perform Botox and dermal filler treatments as part of medical practice when acting within their professional competence and accepted standards of care. Massachusetts classifies botulinum toxin and soft-tissue filler procedures as medical services.
Advanced Practice Registered Nurses (APRN / NP)
Eligible
APRNs may perform cosmetic and dermatologic procedures when appropriately educated, trained, and clinically competent. Massachusetts nursing guidance specifically includes administration of botulinum exotoxins and absorbable fillers among these procedures. APRNs with appropriate prescriptive authority may order medications and treatments subject to Massachusetts prescriptive-practice requirements.
Physician Assistants (PA)
Eligible
PAs may perform medical and therapeutic procedures that are within their competence and within the services their supervising physician can appropriately supervise. Massachusetts requires PA professional activities to occur under physician supervision, although the supervising physician does not need to be physically present in every situation.
Registered Nurses (RN)
Eligible
Massachusetts nursing guidance specifically includes administration of botulinum exotoxins and fillers within cosmetic and dermatologic nursing procedures. RNs must have procedure-specific education and competency and act pursuant to an order from an authorized prescriber. The patient assessment must be completed by an RN or authorized prescriber.
Licensed Practical Nurses (LPN)
Eligible
Massachusetts guidance allows LPNs to perform certain cosmetic and dermatologic procedures, including administration of botulinum exotoxins and fillers, when within their education and competency. LPNs cannot independently diagnose, prescribe, select the medication or dosage, or complete the comprehensive patient assessment; the procedure must be performed under an appropriate prescriber's order and nursing practice structure.
Aestheticians / Cosmetologists
Not Eligible
Massachusetts expressly prohibits cosmetologists, aestheticians, barbers, manicurists, and electrologists from injecting substances such as Botox, collagen, hyaluronic acid fillers, or other injectable products under their cosmetology license.
Medical Assistants / Unlicensed Individuals
Not Eligible
Medical assistants and other unlicensed personnel cannot independently perform Botox or dermal filler injections. Massachusetts medical regulations generally prohibit delegation of invasive medical procedures to individuals who are not licensed to perform them. The state's specific medical-assistant injection exception concerns immunizations, not cosmetic Botox or fillers.
(State requirements are subject to change. Always verify current regulations with your respective Massachusetts licensing board).

What Kind of Training is Required for Botox Certification in Massachusetts?

In Massachusetts, Botox training is particularly important because botulinum toxin and dermal filler injections are classified as medical procedures. Providers must have the appropriate professional authority and obtain the education, clinical competency, and hands-on skills necessary for the procedures they perform.

A strong Massachusetts Botox training course should cover facial anatomy and physiology, patient assessment, indications and contraindications, pharmacology, injection technique, product safety, informed consent, documentation, adverse-event management, and emergency procedures. Massachusetts nursing guidance specifically requires nurses performing cosmetic procedures to obtain education that includes a supervised practicum and competency evaluation appropriate to the procedure.

Massachusetts also establishes particularly clear requirements for dentists. A general dentist using botulinum toxin must complete at least eight hours of training in botulinum toxins, while dermal filler use requires at least eight hours of dermal filler training. Training must include head and neck anatomy, neurophysiology, patient selection, pharmacological effects and contraindications, complication management, informed consent, and hands-on administration. The program must be accredited through ADA CERP, AGD PACE, or another nationally recognized entity approved by the Board.

For Massachusetts providers, receiving a Botox training certificate does not by itself create legal authority to inject. The provider must also hold an appropriate professional license and comply with the prescribing, ordering, supervision, scope-of-practice, and facility requirements that apply to their profession.
