Botox Lip Flip Injection Sites

Summary

A Botox lip flip uses small, carefully placed injections around the upper lip to relax selected orbicularis oris fibers, creating subtle lip eversion without adding volume. Because the same muscles affect smiling, speech, drinking, and lip seal, precise anatomy knowledge, conservative dosing, and proper patient selection are essential to achieve natural results and reduce complications.

A good lip flip should be hard to spot. The upper lip simply looks a little more visible when the patient smiles. Reaching that result starts with precise Botox lip flip injection sites, since the same muscle controls speech, drinking, and lip seal.

Copied maps can quickly produce poor outcomes. Treatment requires a trained, licensed provider with the anatomy knowledge and careful technique.

What Is a Botox Lip Flip?

A Botox lip flip uses a small amount of botulinum toxin to relax selected orbicularis oris fibers. As the inward pull eases, the upper lip can turn slightly outward and reveal more vermilion during a smile. The procedure changes muscle movement without adding lip volume, so lip flip results should remain subtle [1] [3].

Patients may confuse this procedure with filler during consultation. Lip filler and a lip flip can both make the upper lip more visible, but lip fillers add shape and support while Botox changes muscle activity. Some lip flip treatment plans use both.

Important: Lip flip injections are an off-label cosmetic use of Botox in the United States [2] [6]. The consent process and clinical record should identify the off-label use clearly.

What Muscle Is Treated During a Lip Flip?

The orbicularis oris muscle is the main target. This circular lip muscle surrounds the mouth and helps the upper and lower lips close, purse, speak, drink, and form facial expressions [2] [4].

Selective treatment should reduce the inward pull while preserving deeper function.

Broad or deep muscle relaxation may weaken lip control instead of improving lip shape.

Where Is Botox Injected for a Lip Flip?

Treatment targets selected orbicularis oris fibers around the upper lip. Exact placement depends on facial anatomy, lip movement, baseline asymmetry, and the intended result [1] [2] [4].

Provider assessment should include the lips at rest, during speech, and through a full smile. Philtral shape, dental show, baseline asymmetry, existing lip seal, and the degree of inward curl all affect the final injection points.

Upper Lip Flip Injection Sites vs. Lower Lip Treatment

Most Botox lip flip injection sites focus on the upper lip. Lower lip treatment is a separate clinical decision because nearby lower-face muscles affect expression, symmetry, and oral competence. More lip injections may increase lip weakness without improving the result [1] [2] [5].

For that reason, the number of injection sites should never be treated as a measure of treatment quality. The plan should use only the sites needed for the movement pattern observed during assessment.

Botox Lip Flip Injection Sites and Facial Anatomy

  • Orbicularis oris: Selective relaxation can expose more vermilion without disabling the whole lip muscle [1] [2].
  • Cupid’s bow: The central lip shape guides symmetry, but movement may differ between sides [2] [3].
  • Vermilion border: This lip line is a useful landmark, not a universal injection template [2] [4].
  • Philtrum area: Philtral width and column shape affect central upper-lip movement.
  • Elevator and depressor balance: Inward rolling must be separated from weak lip elevation.
  • Nearby smile muscles: Toxin spread can cause smile asymmetry or an uneven smile [1] [2] [5].
  • DAO and lower-face muscles: These muscles affect the mouth corners and require a different treatment plan.

Why Lip Flip Injection Placement Matters

A few millimeters can change appearance and function. A Botox injection placed too deep, too lateral, or across too many injection sites may disturb speech, smile balance, or lip seal. Excess relaxation can also make whistling or drinking from a straw harder [1] [2] [5].

AAFE founder and president Dr. Louis Malcmacher describes the approach as

“Anatomy is everything. Technique is everything.”

Lip Flip vs. Lip Filler: Different Sites, Different Goals

Treatment Primary target Expected result Best fit
Botox lip flip Superficial muscle activity Subtle eversion without true volume gain Upper lip curls inward during smiling
Lip filler Lip structure and tissue support Added lip volume, shape, or definition Thin lips, volume loss, or structural asymmetry
Combination treatment Movement and structure Controlled eversion with added support Muscle pull and volume loss are both present

Who Is a Good Candidate for a Botox Lip Flip?

Patient selection starts with function. A desirable cosmetic result does not outweigh concerns about speech, oral competence, or an already weak lip seal.

More likely to benefit Requires caution or another treatment plan
Upper lip rolls inward or disappears when smiling Goal is dramatic lip volume or much fuller lips
Adequate lip tissue with limited visible vermilion Weak lip seal, speech concerns, or poor oral competence
Wants subtle change without dermal filler Neuromuscular disease or prior poor Botox response
Has realistic expectations Pregnancy, breastfeeding, or a medical concern requiring review [5] [6]

Botox Lip Flip Results Timeline

Time What the provider may observe
Same day Little visible change; brief redness, tenderness, or swelling may occur [5].
Days 3 to 7 Early softening and upper-lip eversion may begin [2] [5].
Days 7 to 15 Lip shape, speech, smile balance, and lip seal are easier to assess [2] [3] [4] [5].
Months 2 to 5 The effect gradually fades, although duration varies [5].

In a 2026 prospective study of 17 women, upper-lip height increased modestly at day 15 without a measurable rise in volume. Measurements then moved back toward baseline by days 30 and 90 [3]. A follow-up near two weeks gives the provider time to assess appearance and function after the initial Botox effect develops.

Botox Lip Flip Side Effects

Brief swelling and sensitivity may improve within 12 to 24 hours, while numbness or functional changes may last longer [3] [5]. A topical anesthetic (numbing cream) is often unnecessary for this short procedure, although it may be offered based on patient comfort and office protocol [5].

Temporary difficulty whistling or drinking through a straw has been reported and may take about two weeks to resolve in some patients [1] [2]. Greater muscle relaxation can affect speech, drooling, smile symmetry, or lip seal until the toxin effect fades [2] [5]. Any speech or swallowing change needs prompt clinical review [5] [6].

Botox Lip Flip Gone Wrong: What Can Happen?

Poor placement, over-relaxation, or toxin spread can cause asymmetrical lip movement, weak upper-lip control, smile imbalance, or little visible result [1] [2] [5]. Botox cannot be dissolved, so prevention and clear pre-op documentation are especially important [5].

The treating provider should document the onset and affected movement, compare the change with pre-op photos or video, and avoid a quick corrective injection before the cause is clear.

How Many Injection Sites Are Used for a Lip Flip?

There is no universal number of lip flip injection sites or dose. Each treatment must reflect the patient’s anatomy, muscle activity, lip function, asymmetry, treatment history, and desired result.

Published clinical reports commonly describe two to four superficial upper-lip injection points and about 4 to 6 total units of onabotulinumtoxinA [1] [2] [4].

Units are not interchangeable across botulinum toxin type A products [6]. Facial anatomy, orbicularis oris strength, asymmetry, prior treatment, lip movement, and the desired result must shape every Botox lip flip procedure.

How Much Does a Botox Lip Flip Cost?

At 4 to 6 units and roughly $12 to $20 per unit, the product portion works out to about $50 to $120. The all-in cost of a Botox lip flip is often closer to $150 to $300 once the consultation, provider fee, office minimum, and follow-up are included [5]. Premium markets may charge more.

For a practice, the small amount of product should not hide the clinical work behind the procedure. Pricing should account for assessment, consent, documentation, the injections, and post-treatment review.

Why Provider Training Matters for Lip Flip Injection Sites

The treatment may take minutes, but the result affects speech, eating, expression, and patient trust. Preventable complications also consume chair time and can weaken confidence in a new facial esthetics service.
